OAH Forms

Notice of Hearing

State Agencies and Local Units of Government use the Notice of Hearing Form to initiate contested cases and due process hearings. Please note that the forms are provided with information that is not required for all notices. The entity preparing the forms is expected to edit the form to provide accurate informaton to the parties. Each document is available in HTML (for viewing with a browser) and Word (for downloading).

Subpoenas

Parties can compel the attendance of witnesses at hearings and depositions with subpoenas. For those unfamiliar with the process, OAH has provided an information sheet. Please review this information if you have any questions as to how to go about obtaining or using a subpoena. The Request Form is available in HTML (for viewing with a browser) as well as Word and PDF (for downloading and printing). The subpoena process for workers' compensation claims is different. Use the process described in the Subpoena Powerpoint (web version) when obtaining a Workers' Comp subpoena, or call (651) 361-7900.

Party and Witness Information

Parties not represented by counsel may find the Pro Se Guide helpful. If you have received a subpoena (or have agreed to testify in a proceeding) please review the Witness Guide.

OAH Video Guide to Hearings

The Office of Administrative Hearings, together with Senate Media, has produced a video guide to the administrative hearing process. The video provides a brief overview of the process and shows examples of offering evidence, questioning a witness, and making final argument.
