In the Matter of the Order of Conditional License and Order to Forfeit a Fine against the License of Lisa Laszcwski

RECOMMENDATION

OF DISMISSAL

 

 

          WHEREAS, the above matter is pending before the undersigned Administrative Law Judge pursuant to a Notice of and Order for Hearing signed on April 25, 2008, by Jerry Kerber, Director, Division of Licensing, Minnesota Department of Human Services;

          WHEREAS, the Notice of and Order for Hearing scheduled the hearing for June 27, 2008, in Cambridge, Minnesota;

          WHEREAS, the parties informed the Administrative Law Judge prior to the start of the hearing on June 27, 2008, that they had been able to reach a mutually agreeable resolution; and

          WHEREAS, the parties placed the terms of their agreement on the record on June 27, 2008;

          NOW, THEREFORE, IT IS HEREBY RECOMMENDED that the above matter be dismissed with prejudice.  The official record shall be returned to the Department of Human Services.
